Lincolnshire County Council's Adult Social Care Hospital Team is seeking Level 1 and 2 Community Care Officers to join our team of Social Workers in enabling and facilitating safe and timely discharges from Lincoln County Hospital, Pilgrim Hospital Boston and Grantham Hospital. To be successful in these roles candidates must have relevant experience working in a health or social care position. They will need to be creative and passionate about supporting people in a person centred way. Be able to work in a multi-disciplinary team and take responsibility for an allocated caseload under the direction of a line manager/supervisor. The ability to work on their own initiative and to reflect on their practice, ensuring continuous professional development, along with good verbal and written communication skills is essential. Applications are invited from people who can demonstrate experience in areas such as; Valuing people using strength based models to assess and plan support with people. Identify eligibility for Continuing Health Care funding and supporting people through the CHC process. Seeing Direct Payments as a way of offering people more choice and control in their lives. Understanding the principals of the Mental Capacity Act and Best Interest decision making. Working creatively, using technology to assess, plan, implement and evaluate people's aspiration and goals. Supervision and training will be offered to the successful candidates to improve their current knowledge, skill and confidence. The teams consist of Community Care Officers, Social Workers and a Lead Practitioner.
