Be able to work in local and multi-disciplinary teams and take responsibility for an allocated caseload under the direction of a line manager/supervisor. The ability to work on their own initiative and to reflect on their practice, ensuring continuous professional development, along with good verbal and written communication skills is essential. Identify eligibility for Continuing Health Care funding and supporting people through the CHC process. Seeing Direct Payments as a way of offering people more choice and control in their lives. Understanding the principals of the Mental Capacity Act and Best Interest decision making. Working creatively, using technology to assess, plan, implement and evaluate people's aspiration and goals. Supervision and training will be offered to the successful candidates to improve their current knowledge, skill and confidence. The Adult Care team provide support for older adults (65) and people with a physical disability and their carers. Promoting people's independence, recognising and celebrating the person's strengths, utilising community resources and social networks to enable the person to live a full, rewarding life, is the key objective. Completing Adult Care Assessments and planning Care & Support for vulnerable people in the Boston area requires practitioners to be able to travel to people's homes, care homes and sometimes hospital settings, however we are currently working remotely where possible.
